iOS folders--rename? delete?

I can create a folder and move titles "into" the folder. But how do you:
1) Rename a folder
2) Delete a folder
3) Move titles back out of a folder

OK, so I figured out rename and delete. But still can't move items out of a folder (or folder to folder).

Somehow, by a somewhat "random" sequence of edit and tapping, I moved a file out of a folder. So it can be done, but I can't repeat the sequence again. If anyone moves files out of folders and knows how they do it, it would be great if they could share the secret! Thanks.
