TBH, I have never tried to play a DVD/BD in any other player than VLC so far. I recently did, and it turned out that e.g. Media Player Classic has no problems in handling both "click to play/pause" and DVD menus as well ... so it appears to me that the issues you were describing above are limited to VLC due to its individual implementation.

Well yeah, that is obviously an implementation issue. I don't think anybody ever claimed that this was fundamentally unreconciliable.

However, the simplistic solution would break support for DVD/BD menus, which is unacceptable. And meanwhile, nobody has stepped up to implement the more advanced solution that would retain support for menus.

Neither did I claim someone did ... but I got the impression that most people asking for the feature aren't aware of that specific detail and therefore can't comprehend wth is the actual problem, while other players don't have issues with it.

For now I've switched to MPC just for the pause/play by clicking on the window.

I use both ... most people might not notice anymore nowadays, but the CPU load caused by VLC is also tremendously higher than that of MPC. Not sure if that's really necessary.
