I think your question 1 is the answer to question 2, mozilla plugin as it is now is pretty much useless.
I finally got it to work today with firefox3.5 and sadly its not a good alternative for quicktime or wmp plugins:
it plays videos but there is no controls, only thing that works is double click for full screen, it plays sounds but they don't loop as intended by the web page even after the player is set to loop everything so its useless for ytmnd.
Don't know if its all bugs or features but I hope mozilla plugin gets in shape soon.

VLC plugin has good Javascript API, you can make your controls with it. WMP and QuickTime can't play satellite TV streams...
