UDP to UDP

I Have spent hours searching google I can not figure this out. I need to be able to transcode and stream a UDP I receive. So pretty much say I receive 224.0.0.1 it is around 5000 KBS H.264, I need to be able to real time take it transcode it to around 800 KBS H2.264 and re Broadcast it out lets say 224.0.0.2. My users have limited bandwith and I can not adjust the UDP size coming in
