I don't know how wide spread these are, but here are a couple if things I found when creating subtitles. When creating original subtitles, number the lines in 10s which leaves room for additions without interfering with line numbers. When working with ready made subtitles, if you find a spot that need an extra line (say after line 21) add line 21.1 and write the line as usual. Hope this is helpful.
