Apparently some graphics cards don't support video overlays on TV outputs... could you try to disable this feature ?

If you have a newer nvidia-card there is a option in their drivers to show overlayed video in fullscreen on the tv output. works like a dream. if any program (like vlc) makes an overlay video output. the tv-out automaticly blows the video out in fullscren on the tv-out.
Can someone make the right TV out

Try changing primary and secondary screen. On laptops this is a common problem.

Try to install the latest Forceware for nVidia gfcards.
They have specifical v-out settings, putting the fullscreen video-out overlay directly on the secondary v-out channel of your card.
Once installed, tight click on the taskbar icon and select TvOut/Analog Monitor as preset.
It worked fine to me even with older nVidia cards...

Disabling overlay (as stated before) should do the same...
