Issue with title/media information

What does the Media Information dialog draw its data from? (standard hotkey Ctrl-i) In particular, I'm concerned about Matroska mkv files. I've reviewed the files with matroska's shell extension and found no metadata attached, so how is VLC coming up with a title for the file in question? Is this something I don't understand about the matroska spec?
On a related note, how can I make the title bar show the file name rather than the media's title metadata? Is this an option?
