Pixelated playback no matter what I try

Windows 10, NVidia Quadro FX380 video card.

Playback is almost always pixelated. It usually goes through about a 3 second cycle of sharp, pixelated, more pixelated, back to sharp. Often the color balance is way off too. I've tried all the usual suggestions: different output module, GPU acceleration on/off, overlay on/off. No impact from anything I try.

The same files play perfectly in Media Player Classic. This happened recently, with VLC's latest series of updates. It used to work beautifully. I've spent two months, off and on, trying to find a solution.
