Modify/Remove playlist items fails - VLC 2.2.4

Variations on this theme seems to be a problem that never goes away!

I copied many mp3s into a folder, several down from "My Music".
The mp3s were named "Track 01.mp3", "Track 02.mp3" etc. They displayed
and played OK from the VLC playlist.
Then within the folder containing the mp3s I renamed the files "A.mp3", "B.mp3"etc
Within VLC, the playlist still contained "Track 01", "Track 02" etc.
Even though the files had new names within the folder, clicking on
"Track 01" within the VLC playlist still played the data originally associated
with "Track 01.mp3", but which was now held in "A.mp3".
Restarting the PC and then starting VLC and watching the playlist, the new file
names ("A", "B", etc) were displayed first, but within a few seconds were
overwritten by the old names ("Track 01", "Track 02", etc).
No variation of "Delete/Remove Selected" would remove the names "Track 01"
etc from the VLC playlist.

a) How is a list of items in a playlist removed from VLC 2.2.4?
b) How is a list of items in a playlist renamed in VLC 2.2.4?
