VLC just now nearly deafened me!!!
I dunno if this is a Mac only problem or what but to recreate it....
* Have it set in prefs to output AC3 through spdif
* Play a video with AC3 audio.
* Quit VLC
* Play a video with standard stereo audio
It will default to 400% volume and make you jump out of your skin in shock!

Are you sure you don't have an audio filter activated like normvol ?

I haven't even changed any other setting but "use SP/DIF when available". And just to make sure I have (a few times) deleted all references to vlc in ~/Application Support and ~/Preferences.
Still happens though.

I have this same problem... It's VERY annoying. This happens on both my Mac Pro and Mac Mini when I use SPDIF. When I use the analog out, the volume issue goes away.
What I see is that the volume jumps to 400% whenever AC3 or DTS is used. (Specifically, this is when VLC sends AC3 or DTS to my reciever.)
For example, I just played a DVD that uses PCM in the menu. When the DVD started, the music was loud and distorted, so I turned the volume down to 100%. As soon as I started playing, the soundtrack was AC3, so VLC's volume jumped to 400%. My receiver indicates that it's getting AC3 and there is no distortion.
VLC's default volume changes whenever I do AC3 or DTS over SPDIF, so if I set the default volume in Preferences, it changes back.
(For those not familiar with how AC3 and DTS work, they send encoded audio through the SPDIF to the receiver. Both AC3 and DTS rely on the stream being bit-for-bit exact, so if VLC tries to do any form of processing on the stream it will result in static. Therefore, VLC can not alter the volume when sending AC3 or DTS over the SPDIF.)
Again, this is very annoying.

If anyone has access to Windows, can you see if that happens there too? Also could you please open a ticket to track this.

I have a similar problem and I'm running Windows 7 64bit.
The volume defaults to 175%, however, not 400%.
Let me know if I can provide further information.

I had the same problem on Mac OS X as well
I went to ~/Library/Preferences/VLC and edited "vlcrc"
Look for this line:
And there should be a line below it that defined the default volume (it may be commented out, in that case remove the # in front of it)
Mine was set to 125%, I changed it to 100% and that fixed it for me.

You can also set this option in the Audio category of VLC's advanced preferences (switch from "basic" to "all" at the bottom left of the Preferences window to see it).
