webm file conversion to mp4 not working

I am trying to convert webm files to mp4 files but everytime I try it strips the audio and force converts it to m4v files. from the drop down to select which file type you want to cover to I'm selecting 'Video H.264 +MP3 (MP4)'. can anyone suggest what I should do.

I'll try to reproduce. Note that MP4 and m4v are basically the same. However, encoding the audio should work nevertheless.

I'm having same problem on my MacBook pro. The problem is the m4v format will not upload to either my website, nor YouTube. Any ideas out there?
