I think, folders can be created only for the media library. There a click with the right mouse in the playlist -> context menu -> create folder.

This is possible with Preferences -> All -> Audio -> Audio output volume step

You have version 2.1.1 Rincewind. It's the latest version.

Uninstall VLC and deletes settings and cache.
Install VLC new, also delete settings and cache.

Try a value between 0.1 and 5.0

what version of VLC have you, 32 or 64 bit?

The place for the libaacs.dll is right, if you use the version for 32-bit.

The place for the KEYDB.cfg is right.

Is the KEYDB.cfg current?

With files from here, the playing from BD works on my pc very well.
