Improve/overhaul DVD menu support

I think it is time to overhaul the code dealing with DVD menus. in my experience, DVD menus rarely work VLC. At times you can navigate and play, though most of the time you cannot properly navigate through the menu and/or play videos. I currently use VLC 0.8.6d for windows, and the same version for linux.
